Once we receive your form it will be reviewed for completeness, we will contact you if we require any additional information. Please make sure all the contact information you filled in is complete and accurate as this could cause a delay in the authorization process. After your application has been reviewed it then goes to our Gifting Committee for a recommendation to the CFK Board of Directors. You will be notified of the decision.
